[The Hub] Allow for comments on Global List of Allowed/Blocked IPs

For automatic blocking of IPs, it would be nice to have a comment next to the IP explaining why it was blocked.

Similarly for Global Allowed IPs it would be nice to add a comment so we can indicate who the IP belongs to – for example – WPMUDEV or CloudFlare
